South County Street Hockey League

Spring 2004 Registration Information

Good news! Everyone can now register for the South County Street Hockey League via the mail. Just follow the instructions on this page to find the right registration form, print it out and mail it in with your payment. The season starts soon so please mail your form in asap. Click here to check for start dates and times. We will accept registrations at the rink, but this may delay your participation while we process the information and some division might fill up so don’t wait!

Download the following form and print it out.  Follow the instructions at the top of the form to for mailing information, etc.  You will need Adobe Acrobat Reader to view and print the registration forms. You may download this program for free by clicking on their logo.

Registration Session

If you are under 18, you must bring a parent or guardian to the registration session.

March 16th and March  18th, 6:30 to 7:30pm. Both registration sessions will be at the South Kingstown Neighborhood Guild.

And any player under 18 needs to complete a Medical Information Sheet. Print it out and bring it with you to speed things along.

All checks should be made payable to the “South County Street Hockey League” or “SCSHL”.

Adult Street players must fill out the SCSHL Adult Street Hockey Registration Form. Make your check payable to “SCSHL.”  The Fall 2003 fee is $20.

All Adult Inline  players must fill out the. Adult In-Line Registration Form. Make your check payable to “SCSHL”. The Fall 2003 fee is $75. Print the form, fill it out, and mail it to the address above NOT TO USA HOCKEY!

All players under 18 must fill out two forms, the USA Hockey Inline Registration Form and the Medical Information Form. Make sure you fill out both pages of the form! Be sure to have a parent or guardian sign both forms or they will be returned to you. Make your check payable to “SCSHL”.

·Instructional InLine Division Registration Form – ages 6 – 8   $25 Print the form, fill it out, and mail it to the address above NOT TO USA HOCKEY!

·Junior InLine Division Registration Form – ages 9-12   $45 Print the form, fill it out, and mail it to the address above NOT TO USA HOCKEY!

·Senior InLine Division Registration Form – ages 13-17   $45 Print the form, fill it out, and mail it to the address above NOT TO USA HOCKEY!

The league needs volunteers. We need help running the clock, recording game statistics, refereeing the games, coaching, and just about everything else. You don’t need to know anything about hockey to help. Right now each of our youth divisions is in jeopardy of closing because we do not have enough participation from the players’ parents. Please consider signing up as a volunteer to ensure that all divisions remain open. Please print out this form, fill it out and mail it to us so we know that you are interested and we’ll be in touch.
